The Approaching Adulthood Team

The Approaching Adulthood Team support young people with complex needs to plan the services they will need once they reach 18.

Needs are assessed on an individual basis and support planned accordingly. Some young people may not be eligible for support from Adult Social Care, Health & Housing (ASCHH) and may be referred on to voluntary services to access other forms of support.

If you have any questions or concerns relating to the transition process you should speak to the professionals you are currently working with (i.e. SENCO, Family Support Workers) or contact the Approaching Adulthood Team on 01344 354466 or email: approaching.adulthood@bracknell-forest.gov.uk
