Our Community Connectors work with a variety of residents aged 18+, from older people to those with learning disabilities or autism, helping them maintain independence, lead fulfilling lives and stay physically and emotionally well. They do this by helping people identify what they are struggling with and the support they may need to overcome this. They then work with them to search for and connect with appropriate local groups and voluntary organisations which could help to meet their needs.
From social groups or activities to health and wellbeing services and local faith or community groups, we work with a range of organisations to connect people with services which can have a positive impact on their lives and help them overcome any problems they are experiencing. Community Connectors typically work with people over a period of around six weeks, helping to research the right groups or activities for them or accompanying them to the sessions until they feel confident enough to attend by themselves.
The Community Connector service aims to ensure people get the most out of life and prevent their circumstances worsening to a point where they require higher level social care or health services.
